How to refer a Patient for admission during our building project from June 2012 until July 2013.
The Hospice’s 25 year old In-Patient unit is being extended and refurbished during the 12 month period from June 2012 to June 2013. Click here to find out more about this exciting project.
If you need to refer a patient for Hospice In-Patient care during this time, our own experienced and specially trained clinical staff will be able to care for them in a dedicated hospice unit at Ward 11, Huddersfield Royal Infirmary.
All care will be provided by Hospice medical, nursing and other clinical staff and the service will remain totally separate from the rest of the NHS services at the hospital.
Kirkwood Hospice will be fully responsible for patient care and treatment during the admission. We will continue our existing service using our own catering and cleaning staff and our dedicated volunteers will continue to support our staff to make the patients stay as comfortable as possible.
Our eligibility criteria, referral procedures and admission policy remain unchanged during the period of the building project. For more information please visit the pages detailed in the left hand panel of this page.
To make a referral for any Kirkwood service please contact the Community Palliative Care Team office on 01484 557900 as usual or refer directly to the relevant department. Alternatively you can download our referral form from the panel to the left.
For patients in hospital, referrals should continue to be made to the hospital Palliative Care Team who will assess the patient and make any necessary onward referrals to the hospice.
All our other services to support you and your patients and their families will remain fully operational on our main hospice site at Dalton as usual. Click here for information on these services
Information leaflets for patients and visitors about our care whilst at ward 11 will be available to download shortly, together with some photographs of the hospice ward. These may help you when discussing possible admission with patients.
Please contact us if you have any difficulties in accessing our services at any time. In the first instance contact a member of the Community Palliative Care Team (click through to their page) on 01484 557900 Monday to Friday 8.30 to 4.30pm or you may wish to use our 24 hour advice line.
