Community Palliative Care

Providing specialist advice and support to those, living in Kirklees, who need it.

What We Offer

We work in partnership with GPs and Community Nurses when specialist palliative care advice and support is needed. We are able to visit you in your own home and are also contactable by telephone.                                                              

We also work in partnership with your hospital consultants and specialist nurses.

We work Monday - Friday, 8.30am - 4.30pm. Outside of these hours, advice is available by contacting our 24 Hour Advice Line on: 01484 557910


Our Team

We have a team of experienced, specially trained nurses, a consultant in palliative medicine and a secretary. We are based at Kirkwood Hospice in Huddersfield.  

You are welcome to contact the team to discuss a referral. 


How We Can Help

  • Specialist advice on all aspects of controlling pain and other symptoms
  • Emotional support and assessment of your needs
  • Advice about benefits and other social issues
  • Referral to a physiotherapist, occupational therapist or social worker if necessary
  • Support for your family and carers
  • Information about your diagnosis and treatment

In order to provide seamless care, the specialist nurse who visits you will need to share information about your care with other healthcare professionals that you know.

The nurse will discuss this with you on the first visit and ask you to sign a form giving your consent.

We know that meeting one of our nurses can be confusing, as some nurses in other areas and in hospitals are often sponsored by Macmillan Cancer Support. Our nurses do the same role as Macmillan Nurses, but in Kirklees, it is Kirkwood Hospice who provide and pay for specialist palliative care nurses.
